APPLICANT REQUESTS THAT:

Her Home of Record (HOR) be changed to Albuquerque, NM.

APPLICANT CONTENDS THAT:

She no longer has ties to Cleveland, OH and wants to start her 
retired life in Albuquerque, NM.

STATEMENT OF FACTS:

The applicant is on active duty in the Regular Air Force and is 
serving in the grade of Staff Sergeant (SSgt, E-5).  

According to DD Form 4, Enlistment/Reenlistment Document Armed 
Forces of the United States, dated 8 Feb 01, her HOR is 
Cleveland Hts, OH.

AIR FORCE EVALUATION:

AFPC/DPSIPE recommends denial.  After a thorough review of her 
military personnel record and other supporting documentation, 
there appears to not be an error or injustice in determining her 
HOR.  In Accordance With (IAW) the Joint Federal Travel 
Regulation (JFTR), volume 1, Appendix A1, part 1, the DD Form 4, 
Enlistment/Reenlistment Document Armed Forces of the United 
States, is the source document for HOR and Place of Entry (POE).  
The place recorded as the home of the individual when 
reinstated, reappointed, or reenlisted remains the same as that 
recorded when commissioned, appointed, enlisted or inducted, or 
ordered into the tour of active duty, unless there is a break in 
service of more than one full day.  Only if a break-in-service 
exceeds one full day may the HOR be changed.  The HOR is the 
location identified by the individual upon entering the service, 
not a location where the individual is later assigned or a 
location selected for convenience; a correction must be fully 
justified.  Domicile or legal residence may change, but the HOR 
will not.  The AFPC Enlisted Accessions Branch confirmed the 
applicant’s DD Form 4/1 dated 8 Feb 01, as well as subsequent 
enlistment documents list Cleveland, OH as the HOR.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

On 24 Jan 14, a copy of the Air Force evaluation was forwarded 
to the applicant for review and comment within 30 days (Exhibit 
D).  As of this date, this office has not received a response.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was timely filed.  

3.  Insufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice.  We took notice 
of the applicant's complete submission in judging the merits of 
the case; however, we agree with the opinion and recommendation 
of the Air Force office of primary responsibility and adopt its 
rationale as the basis for our conclusion that the applicant has 
not been the victim of an error or injustice.  Therefore, we 
find no basis to recommend granting the relief sought in this 
application.

THE BOARD DETERMINES THAT:

The applicant be notified that the evidence presented did not 
demonstrate the existence of material error or injustice; that 
the application was denied without a personal appearance; and 
that the application will only be reconsidered upon the 
submission of newly discovered relevant evidence not considered 
with this application.
